Select the option that best describes your evaluation of the crew
of officials. This evaluation should be based solely on the
performance of the crew during the one game you identify below. Any
previous experience with the crew, or the individual officials
being evaluated should not be taken into consideration.
The five areas to evaluate are:

Appearance -
did the officials appear neatly dressed and properly
equipped?

Hustle -
did the officials "hustle" to get into position to make the
call?

Mechanics -
did the officials give good signals, did they work well as a
team?

Rules -
were the rules correctly, and consistently administered and
applied?

Presence -
were they approachable yet authoritative, did they keep control of
the game?
Please rate the officiating crew's OVERALL performance. Space is
provided near the bottom of the form, for specific comments, or
suggestions you may have.
Your specific comments are especially helpful in
understanding your rating.
